After you receive confirmation of the appointment from us, kindly fax to the appointment location:

• A clear copy of the insured's insurance card (both sides)
• Picture ID (drivers license, school ID, etc.) of the patient

Please bring the following with you at the time of your appointment:

• Email confirmation of the appointment
• Prescription from the doctor
• Insurance card of insured
• Picture ID of the patient

NOTE:

• Pre-Certification or Authorization may be required by some insurances prior to the scan.

• Currently AVAILABLE ONLY at Melrose Park: PET, CT, Ultrasound, Breast MRI and EMG.
